The Community Based Transition Care Program is seeking an experienced and enthusiastic Occupational Therapist to join their team on a Temporary Full Time basis for up to 12 months.

The Community Based Transition Care Program is a mutlidisciplinary team that manages and provides goal orientated and time limited care and services to 30 community packages. The program faciliates a continuum of care for older people who have completed their hospital episode, including acute and subacute care and who need more time and support to make a decision on thier long term care options. TCP aims to optimise the functioning and independence of the older person after a hospital stay within their own home.

The successful applicant will work as part of a multidisciplinary team and possess strong communication, problem solving and time management skills. Supervision of students will also be required. Experience in a community setting is highly desirable with well-developed occupation based assessment and intervention skills, and demonstrate sound clinical reasoning for an older complex population. This position would suit someone who is passionate about providing best patient care in the community.
