Position Summary

This role provides an exciting opportunity to be a founding member and an integral part of a new Outreach service. The Mental Health & AOD Outreach Team is for consumers aged 18-64 years, providing follow up and care coordination for up to 28-days post presentation to the Emergency Department with the aim of reducing the likelihood of a future ED hospital presentations., and ensuring patients are linked in with appropriate community services and supports.

The MHAOD Outreach Team is multi-disciplinary and operates utilising the framework of patient care-coordination. It encompasses Mental Health (MH), Alcohol and Other Drug (AOD) clinicians, Care Coordinators, and Peer Support Workers.

It utilises a framework of person-centred care that is least restrictive and respectful, is trauma-informed, recovery-focused, in line with the principles of harm reduction, and is family and carer-inclusive to deliver Best Care.

In the first instance this role will assist in establishing and implementing process and pathways to support this new outreach model of care. Once the day to day service is fully operational this role will include a clinical caseload to support the team. In addition to this, they will be responsible for providing operational leadership and strategic direction by enhancing and improving the service, managing waiting lists, ensuring strong clinical governance (with professional discipline), staff orientation, delivering improved outcomes for patients and acting as a key liaison for the team.

 Full time or Part-time job share options will be considered.

 To be Successful in this Role you will have:

  • Approved degree in Allied Health (Social Work or Occupational Therapy) with current AHPRA registration or equivalent
  • Relevant post graduate qualification desirable
  • Skill and experience working with consumers with mental illness and/or AOD presentations and their family and carers
  • Strong leadership skills, including proven interpersonal and communication skills
  • Proven ability to lead and supervise staff
  • Highly developed interpersonal, written and electronic communication skills
  • Commitment to continuous quality improvement and professional development of self and others
  • Strong understanding of public health and community services
  • Demonstrated ability to work in multidisciplinary teams
  • Bring a sense of calm and stability in a dynamic and developing clinical setting
  • Contribute flexibly, creatively and enthusiastically to initiatives for ongoing improvement
  • Current Victorian driver’s licence
