About the Role

The Western Health Community Services Division has an exciting opportunity for an experienced Diabetes Educator to join the Chronic Diseases and Complex Care Program. The successful applicant will also work in the Diabetic Foot Service to provide Diabetes Education and support to this patient cohort.

In addition to having a recognised Diabetes Educator qualification, the successful applicant will have high level interpersonal and communication skills, experience in health coaching and the ability to work independently and/or within a multidisciplinary team environment.
