About the Role

Seeking an Exercise Rehabilitation nurse with specific skills in Cardiac Rehabilitation to work part time within the Community Based Rehabilitation (CBR) service.  The Exercise Rehabilitation program is well supported by CBR and incorporates a multi-disciplinary team that provides assessment and therapy to Cardiac, Chronic Heart Failure and Pulmonary Rehabilitation patient cohorts.   This program operates over two sites, Sunshine and Williamstown hospitals, with this position primarily based at Sunshine. This role is well supported by management and administrative teams.

  • Permanent, Part Time 64 hour fortnight
  • Support and education provided for particular clinical areas offered by the Exercise Rehabilitation program

Duties

  • Assistance with the co-ordination of exercise rehabilitation programs with emphasis on the
  • Cardiac and Chronic Heart Failure cohorts.
  • Assistance with the Pulmonary Rehabilitation program within scope of practice
  • Provide expert clinical judgement and assessment with individual patient and group settings
  • Provide expert clinical judgement triaging referrals to the service
  • Assistance with the co-ordination and involvement where appropriate in patient education delivery

Skills & Experience

  • Current APHRA registration
  • Minimum five years post-registration with three years relevant specialist clinical experience
  • Possess excellent clinical and organisation skills
  • Demonstrate exceptional interpersonal and communication skills
