Western Health currently has an exciting opportunity for a highly motivated individual to join Western Health’s Division of Aged Care and Subacute Services in the role of Acting Head of Unit- Rehabilitation Medicine.
The Division of Aged Care and Subacute Services provides quality health care for patients across acute and subacute aged care and rehabilitation services. Aged Care and Subacute Services play an integral role within the health care continuum, supporting patients to maximise their independence and functioning and, in doing so, minimise long-term health and community care needs. This diverse group of services is fundamental in promoting effective and seamless services across the care continuum.
Aged Care and Subacute Services include the inpatient Acute Aged Care, Cardio-Geriatric, Ortho-Geriatric, Geriatric Evaluation and Management (GEM), Rehabilitation, Palliative Care and Transition Care Services. To ensure that patients have appropriate and timely access to these services, the Division also provides a Subacute and Nonacute Assessment and Pathways (SNAP) service to facilitate pathways to the right care setting.
The Western Health Rehabilitation Service currently has a 30 bed rehabilitation specific ward at the Sunshine Hospital campus, as well as rehabilitation beds at Footscray Hospital (co-located in a 20 bed subacute unit) and rehabilitation beds at the Williamstown Hospital campus (co-located in a 30 bed subacute unit).
The Head of Unit role is integral to the leadership team within the Division and is responsible and accountable for the delivery of Best Care and effective clinical governance, operational, and financial leadership across the rehabilitation physician workforce.
To be successful, it is desirable to have a proven track record of effective leadership in a health service and a passion and commitment for the delivery of high quality, safe, patient centred care. In addition to Head of Unit responsibilities, the successful candidate will hold a clinical caseload within the rehabilitation services.
