About the Role
Western Health’s Community Services Leadership Team is seeking an experienced Health Care Professional to lead, manage and support the newly formed HIP (Health Independence Program) Community Services Team.
- Full Time Ongoing role

We are seeking applicants who are passionate about integrated care, patient flow, community based services and have experience and qualifications to lead change and ensure the delivery of person centred, evidenced based Best Care. The Team Leader will provide the day to day management of the multidisciplinary staff in the HIP Community Services Team. The position will suit a person who is looking for a new challenge, enjoys working in a changing and fast paced environment and is committed to providing best patient care.
To be successful in this role we are looking for a leader with the following skills and experience:
- Adaptability to work in an innovative and changing environment
- Excellent communication and organisational skills
- Experience in delivering results in specified timeframes
- A commitment to the delivery of Best Care to our patients
- Willingness to work across silos of care to deliver best care
- Has held a Leadership role in a health related field
- Previous experiencing in the management of staff working across a matrix arrangement
- The management of complex rosters, budgets, leadership tasks such as staff supervision, completion of performance appraisals
