About the Role

The Western Health Renal Service provides a comprehensive range of services which include satellite, acute and home dialysis. The Home Therapies Team are seeking an enthusiastic Clinical Case Coordinator ( 0.9EFT) who will be responsible for the provision and coordination of high quality patient-focused nursing care within the unit and in the home environment. The successful applicant will be required to provide clinical training and care in the home as well as providing education and advice in the inpatient and outpatient environment.

Skills & Experience

  • Have demonstrated leadership ability, skill in patient education and a commitment to quality service provision.
  • Be able to adapt teaching styles for a diverse patient population
  • Have experience preferably in both haemodialysis and peritoneal dialysis. (however it is not essential to be competent in both modalities)
  • Generally work Mon - Fri, but must be available to work some evenings and weekends should patient training require this.
  • Participate in the on call roster

Selection Criteria Essential: Ability to teach a diverse group of patients Peritoneal Dialysis Experience Victorian Drivers licence
Desirable: Post Graduate Cert/Dip in Nephrology Certicificate IV in Education/training Prior experience working in Home Therapies Haemodialysis experience
