About the Role
- Registered Nurse or Allied Health Professional
- Advice Coordination and Expertise (ACE) Inpatient Team
- Temporary Full time replacement position (until October 2019)
- 7 day Rotating Roster including some weekend and evening shifts allowing for a flexible roster
The successful applicant will be working in our Advice Coordination and Expertise (ACE) Inpatient Team, supporting patient care via the delivery of a service model that is committed to the principles of Integrated Care to patients. Inpatient Care Coordinators are allocated to Medical Units within Western Health and are responsible for facilitating access to Western Health Community Services (Health Independence Programs), from inpatient services in order to support the transition from hospital to home. Western Health is committed to streamlining and responding quickly to patient needs, enabling them to easily navigate the health system. As part of this model of care, the ACE Care Coordinator are crucial in identifying clients and working with other services/programs to ensure that clients get the right service mix to safely support care in the community.
We are looking for an experienced Health Professional who is passionate about integrated care and patient flow; and have experience in complex discharge planning.
Skills & Experience
- Appropriate tertiary qualification in Nursing or Allied Health and eligibility for registration with relevant body.
- Minimum of 5 years clinical experience in relevant setting including acute and community.
- Experience and a preference for working in multi-disciplinary teams.
- Experience in assessment of people with complex needs, discharge planning, case conference and care planning.
