The Community Transition Care Program is seeking an experienced and enthusiastic Grade 2 Physiotherapist to join their team on a temporary part time basis for a 12 month period.

The Community Transition Care Porgram is a multidisciplinary team that provides goal orientated and time limited care and services to 30 community packages. The program facilitates a continuum of care for older people who have completed their hospital episode, including acute and subacute care and need more time to make a decision on their long term aged care options. TCP aims to optimise the functioning and independance of older people after a hospital stay in their own home.

The successful applicant will work as part of the mutlidisciplinary team and possess strong communication, problem solving and time management skills. Supervision of students will also be required. Experience in both community and hospital settings is essential. The successful applicant will have well-developed assessment and intervention skills and able to demonstrate sound clinical reasoning for an older complex population. This position would suit someone who is passionate about providing best patient care in the community.
