Western Health is seeking an experienced Health Professional (Allied Health or Nurse) to join a new and dynamic team. We are looking for people who are passionate about integrated care and patient flow and have experience in complex discharge planning. Representing Community Services, the Care Coordinator will have the opportunity to work in areas such as acute, subacute, surgical and maternity and work between Hospital campuses. The successful applicant will build strong relationships with ward staff, Access Managers, and Community Services staff and will play a pivotal role in discharge planning to Community Services. The position will suit people who are looking for a new challenge, enjoy working in a changing and fast paced environment and are committed to providing best patient care. The position is for 6 months from end of January/start of February with ongoing work after this period a possibility.
