About the Role
The Activity Program staying active Coordinator works closely with the Multidisciplinary team providing leadership in the overall activity program to promote wellness, independence, social connections, recreational and rehabilitative activities for clients in the residential based Transition Care Program.
The primary role of Staying Active Coordinator (Hazeldean) is to coordinate the overall activity program at Hazeldean Transitional Care. The role also includes developing and delivering diversional activities in group and individual formats with Transition Care clients.
Duties
Lead the development of individual and group activity, leisure and lifestyle programs for TCP clients
Coordinating and deliver activities programs in partnership with the staying active staff and volunteers
Undertake other duties as required/directed as service needs change or grow
Skills & Experience
As an experienced Activities coordinator/Lifestyle coordinator/diversional therapist, you’ll bring a well-rounded understanding of how to organise, coordinate and manage fun and beneficial activities for our clients. You’ll demonstrate an awareness and consideration of the social and recreational needs of clients,
- Knowledge and experience in the aged care sector
- Relevant vocational/Tertiary qualification in Aged care, Disability, Community Services, Home and Community Care or other relevant field
- Demonstrated experience coordinating and evaluating lifestyle programs and lifestyle teams in an Aged Care setting
- Experience in managing/mentoring staff and volunteers
- Demonstrated high level communication skills
- Ability to promote team work and team building spirit by modelling leadership and team behaviours
- Experience working with clients with a diagnoses of Dementia
- Strong computer skills with Microsoft Office Package
