About the Role

Drug Health Services is seeking a highly motivated, enthusiastic and experienced person to deliver Care and Recovery Co-ordination services to clients referred for both Voluntary and Mandated AOD treatment.

You will be part of a small supportive multidisciplinary team. As the successful candidate you will be committed to person-centred care provision, with excellent client engagement skills and a strong desire to provide a positive therapeutic experience to a client group often marginalised and disenfranchised within our community.

The Care and Recovery Co-ordinator works collaboratively with clients and a range of stakeholders across the health and welfare sector, and the criminal justice system, to reduce risk of harm to clients and the community.

You will be able to:

  • Assess the needs of complex client presentations, supporting clients to develop an achievable treatment plan
  • Implement person-centred care delivery to assist clients to reduce or cease harmful alcohol and drug use
  • Respond to and address Risk associated with harmful substance use;
  • Assist clients to establish healthier lifestyles
  • Assist clients to avoid and better manage relapse by providing linkage to relevant identified in their Individual Treatment Plans
  • Assist clients to be free from or reduce problematic alcohol and drug use;
  • Possess a strong sense of personal accountability, resilience and emotional intelligence
  • Possess strong administrative skills including a good level of computer literacy
  • Liaise with other case workers and relevant stakeholders to support ongoing attendance and engagement in treatment
