A fantastic opportunity exists for Grade 2 Social Workers to work within an innovative team of social workers at Western Health. We have two full time fixed term positions available.

The successful applicant will demonstrate highly developed skills in:

• Complex assessments
• Risk screening
• Supportive counselling to patients and their families
• Timely discharge planning and coordination of care with a diverse range of internal and external stakeholders
• Interpersonal communication
• Information Technology
• Undertaking quality and research activities

Essential Criteria:

• A 4 year Bachelor of Social Work degree (or qualifying Masters)
• Eligibility for membership of the Australian Association of social workers
• Minimum 3 years social work experience
• Current Victorian driver’s licence
• Current Working with Children Check

Desirable:

• Social Work experience in a health setting
