About the Role
- Occupational Therapist Grade 2
- ACE ED Team & Community HIP Outreach
- Full time 12 months temporary (until September 2019) role
- 7 day Rotating Roster including some weekend and evening shifts allowing for a flexible roster and the benefit of 5 weeks annual leave
The successful applicant will be working in our busy Emergency Departments and in Community Services, supporting patient care via the delivery of a service model that is committed to the principles of Integrated Care to patients. Western Health’s - ACE (Advise Coordination & Expertise) Team is a multidisciplinary team providing care coordination services in the Emergency Department and inpatient wards for clients with complex medical issues at risk of presentation and admission. The role is to reduce presentations / admissions and optimise patient care through the provision of Care Coordination; Outreach services (Community HIP) and integrated Care planning.
The ACE Team and Community HIP Outreach Service is focused on building an integrated and supportive framework to strengthen and to promote person centred care. This role gives the successful candidate the opportunity to expand their clinical skills in an innovative and supportive model. The role is primarily based in the emergency department, however will also include a component of outreach work; with both roles involving Occupational Therapy and generic care coordination tasks.
The successful candidate will have commitment to developing integrated services for consumers with multiple and complex service needs that often ‘fall between the gaps’.
Skills & Experience
- Appropriate tertiary qualification in Occupational therapy and eligible for discipline specific registration.
- Minimum of 3 years clinical experience in relevant setting including acute and community.
- Experience and a preference for working in multi-disciplinary teams.
- Possessing initiative and drive to develop and negotiate new work and clinical practices within the health service and wider service system.
- Experience in assessment of people with complex needs, discharge planning, case conference and care planning.
