About the Role

Allied Health Grade 2 / Registered Nurse Grade 4A
ACAS are in search of a highly skilled and experienced clinician to become an integral member of our team and support our growing diverse western community. ACAS Clinicians operate primarily as generalist aged care assessors. ACAS Clinicians conduct comprehensive assessments of a restorative, physical, medical, psychological, cultural and social nature.
Position Summary
Significant relevant experience in the aged and health-care sectors would be essential, as would be the ability to undertake comprehensive assessments using electronic portable devices, and a current Victorian Drivers Licence. The successful applicant must have experience in undertaking assessments of older persons in a community and hospital setting and the ability to prioritise tasks and manage a weekly case load is required.
The position will be based at the Footscray Campus with required flexibility to move across other Western Health campuses.

- Provide comprehensive support in both hospital and community settings
- Working collaboratively with the ACAS team
- Promote Wellness and Reablement
- Ensure safe and high quality care is delivered in line with Western Health values and policies
Essential Criteria
- Bachelor Degree Qualifications in Social Work, Nursing, or Occupational Therapy
- Current Registration with AHPRA is applicable
- Excellent ICT Skills to fulfil this role
- Demonstrated advanced knowledge/assessment/problem solving skills in the area of care coordination for the older person.
