About the Role
We are seeking an enthusiastic and experienced Diabetes Educator to join our dynamic team within the Chronic Diseases and Complex Care Program at Western Health.
You will be based predominately at the Footscray Hospital campus but may be required to move to other campuses as the workload demands.
To be successful, you must be credentialed by the Australian Diabetes Educators Association (ADEA) or working towards. This position involves working in the Diabetes Foot Service (DFS), diabetes outpatient clinic, outreach work & working with the care coordination team to improve diabetes self-management skills & reduce hospital admissions.
