About the Role
Seeking an experienced Grade 2 Clinical Pharmacist to work part time in Pharmacy across Western Health sites with outstanding clinical and interpersonal skills that demonstrate a willingness to develop and maintain their knowledge
- Permanent, Part time 56 hours per fortnight
- Position available from 8 Jan 2018

An excellent opportunity for a driven and experienced Pharmacist to join the Pharmacy team.
Duties
- To ensure patient outcomes are optimized by working to achieve the best Quality Use of Medicines
- Support prescribers and patients to maximize clinical benefits of drugs and minimize unwanted reactions
- Minimise treatment costs by optimizing drug prescribing and supply
- Provide expert clinical pharmacy advice to range of specialties
- Actively participate and support educational and operational activities of the department
Essential Criteria
- Bachelor of Pharmacy or equivalent.
- Eligible for general registration with AHPRA.
- Experience in providing clinical pharmacy services to a broad range of specialty areas.
- Well developed written, oral and interpersonal communication skills, and the ability to work co-operatively with hospital staff and patients.
- Sound knowledge and understanding of all relevant aspects of pharmacy services, particularly a demonstrated ability to practice in accordance with the SHPA Guidelines.
- Experience in contributing to practice improvement projects.
- Experience in supervising and providing education and training to pharmacy staff and students.
- Works with a minimum of supervision.
- Ability to work as part of a team and contributes to projects and initiatives.
- Strives for maximum attendance/punctuality.
- Fosters and maintains working relationships.
- Demonstrated ability to adapt to a changing environment and demonstrate initiative within it.
- Ability to understand and adhere to Western Health Policies and Procedures.
- Active contributor to committee meetings.
- Practices collaboratively as part of a multidisciplinary health care team.
- Demonstrates an ability to identify and solve problems.
- Demonstrated ability to meet time constraints and to determine workload priorities.
- Possesses well-developed computer skills.
