About the Role

Seeking an experienced Grade 2 Clinical Pharmacist to work part time in Pharmacy across Western Health sites with outstanding clinical and interpersonal skills that demonstrate a willingness to develop and maintain their knowledge

• Permanent, Part time 56 hours per fortnight

An excellent opportunity for a driven and experienced Pharmacist to join the Pharmacy team.

Duties

• To ensure patient outcomes are optimized by working to achieve the best Quality Use of Medicines
• Support prescribers and patients to maximize clinical benefits of drugs and minimize unwanted reactions
• Minimise treatment costs by optimizing drug prescribing and supply
• Provide expert clinical pharmacy advice to range of specialties
• Actively participate and support educational and operational activities of the department

Essential Criteria

• Bachelor of Pharmacy or equivalent.
• Eligible for general registration with AHPRA.
• Experience in providing clinical pharmacy services to a broad range of specialty areas.
• Well developed written, oral and interpersonal communication skills, and the ability to work co-operatively with hospital staff and patients.
• Sound knowledge and understanding of all relevant aspects of pharmacy services, particularly a demonstrated ability to practice in accordance with the SHPA Guidelines.
• Experience in contributing to practice improvement projects.
• Experience in supervising and providing education and training to pharmacy staff and students.
• Works with a minimum of supervision.
• Ability to work as part of a team and contributes to projects and initiatives.
• Strives for maximum attendance/punctuality.
• Fosters and maintains working relationships.
• Demonstrated ability to adapt to a changing environment and demonstrate initiative within it.
• Ability to understand and adhere to Western Health Policies and Procedures.
• Active contributor to committee meetings.
• Practices collaboratively as part of a multidisciplinary health care team.
• Demonstrates an ability to identify and solve problems.
• Demonstrated ability to meet time constraints and to determine workload priorities.
