About the Role
The Western Health Community Services Division are looking to form a team of dynamic and experienced Registered Nurses who will become the Care Coordination team for the Chronic Diseases and Complex Care Program.
The Care Coordination team works with adults and children who have multiple chronic comorbidities and complex needs, with the aim of preventing avoidable admissions to hospital. To become a member of the Paediatric Care Coordination team you will have high level interpersonal and communication skills, advanced knowledge and experience in assessment and management of asthma and eczema.
