The role of the Case Manager/Clinical Consultant is to engage with patients, families/carers, and the treating team to identify and assist the patient and family to finalise their ongoing care plan and pathway following a stay in hospital.

The Case manager/Clinical Coordinator performs a critical clinical assessment and communication role in order to match the patient and family/carers care needs with the service system including subacute/transition care, community services for patients going home or with aged care facilities for those going into residential aged care.

Working from a case management framework, the Case Manager/Clinical Coordinator will work with the treating team and foster collaborative relationships with our partners including ACAS. The case Manager will foster strong relationships with aged care providers in order to provide a highly skilled case management service for patients.

The position also encompasses an essential component of the Subacute and Nonacute Access service (SNAP) and forms the frontline interaction with acute treating teams.  The role assists organisational flow and access by identifying patients which may benefit from a subacute or nonacute pathway by quickly responding to referrals from the treating team.
