The Palliative Care Consultancy Service covers all campuses of Western Health providing care in the inpatient, outpatient and community settings, including the interface between community and acute/sub acute settings. 

About the Role

This role is funded for 18 months through a Department of Health and Human Services (DHHS) grant awarded to Western Health in 2018. The PallC-ED Project (Palliative care in the Emergency Department: Improving end of life care in hospital and the home) aims to improve access to high quality end of life care for patients that have presented to the Footscray Hospital Emergency Department (ED) who are dying or have a life-limiting illness. 

The PallC-ED CNC is a pivotal member of the project team and will assist with project design, implementation and evaluation. The role will primarily be based in the Footscray Hospital ED and will involve leading the roll-out of a new palliative model of care that will enable early and appropriate identification, assessment and referral of patients. The CNC will act as the key point of contact for community-based services while being in close contact and consultation with the internal Palliative Care Consultancy Team.

Duties

The CNC will identify, assess and provide expert advice on palliative patients in the Footscray ED, and will also support the project team with data monitoring and other project activities as required. They will engage in difficult end of life conversations with patients and their families, and provide support and information while remaining respectful of their rights, values and preferences. As the key point of contact between the patients, medical and nursing teams, community palliative care services, allied health and GPs, the CNC will coordinate patients’ wishes to return home for end of life care. The role will also support the multidisciplinary team through the design and facilitation of education sessions in palliative care. 

Skills & Experience OR Essential Criteria

  • Registered Nurse
  • Previous experience working in Palliative Care (3 years minimum post-graduation)
  • Have completed or be working towards a Masters in Nursing or Post Graduate Diploma in Palliative Care/Oncology or relevant area of specialty
  • Prior experience in project delivery and quality improvement
  • Established advanced clinical decision making skills in nursing, palliative care assessment and care planning.
  • Demonstrated ability to work autonomously and as part of a team
  • Proficient skills to influence, educate and support staff
  • Excellent communication, liaison, negotiation and interpersonal skills
