About the Role

Clinical Nurse Consultant – Heart Failure Expressions of Interest

Mat Leave – Relieving position (Temporary until 18th October 2020)

An excellent opportunity for a motivated, and experienced cardiac RN with a keen interest in heart failure to join our team.

The service aims to improve clinical outcomes for older patients that present with heart failure, minimise functional decline and adverse events and improve nursing practice and the coordination of care for this patient group. The Clinical Nurse Consultant role is integral to this service and supports consistency in patient care across the continuum of care.

Duties

  • Co-ordinating the patients care from admission to discharge including daily review of inpatients with geriatrician and cardiologist and cardiology medical team on plan of care
  • Working collaboratively as part of a multi-disciplinary health care team
  • Supporting the ward nursing staff with the nursing management for these patients
  • Liaising with the SNAP team
  • Patient and carer education and discharge planning
  • Discharge follow up including telephone assessment & clinical evaluation and review of patients in the weekly heart failure outpatient clinic.
  • Co-ordinating the rapid access clinic in collaboration with the consulting cardiologist
  • Act as a resource and liaise with community heart failure nurse facilitators (HARP / Health links), General Practitioners, community nurses and the community palliative care team.

Skills & Experience OR Essential Criteria

  • Successful completion of a Bachelor of Nursing qualification or equivalent
  • Successful completion of postgraduate qualification in Cardiac Nursing or equivalent
  • Current registration as a Registered Nurse in Australia
  • Possess excellent clinical, organisational, leadership and communication skills
  • Be self-directed and adaptable and ability to work autonomously
  • Have a keen interest in aged care nursing.
  • Have the alibility to be released from current substantive position.
