Position Summary

Western Health is seeking Nurse Coordinators to work within the Pathway to Good Health Program, as part of its Integrated Community Health Service. These are, permanent position with both part time and full time opportunities available to the right candidates. Working within a dynamic and diverse multidisciplinary team and in collaboration with key stakeholder you will be supporting Children in Out of Home Care (OOHC) to access comprehensive health supports and address significant unidentified and unmet health needs.

You will be supported to progress your career with high quality supervision and professional development opportunities by working alongside medical and allied health clinicians from our specialist programs and clinics.

To be Successful in this Role you will have:

  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered though the Australian Health Practitioner Regulation Agency (APHRA)
  • Extensive paediatric nursing experience, including with children at risk of abuse or neglect, and their families.
  • Proven competence in advanced clinical and leadership skills related to coordination of patient care and management of a clinical service.
  • Excellent organisational, interpersonal and communication skills
  • The ability to work collaboratively as a member of a multidisciplinary team
